It can be challenging to remember which questions you want to address with your doctor when you’re at a doctor’s appointment. Diabetes is constantly changing and it’s easy to forget questions you’ve had in the past when you are actually in the office. To help you stay on top of everything, use this handy checklist that you can print out.

Before your next appointment, ask yourself the questions on the list. If you’re not sure what the answer should be OR if you have more questions about that topic, that’s something you should talk with your doctor or diabetes educator about. This list will help you get the most out of your time with a doctor or educator by keeping you focused and on track. Click here to download and print your checklist.
